The Patient Care Coordinator role supports physicians, dietitians and patients and is critical to our company mission of outstanding clinical care.</p> <p><strong>About the role:</strong></p> <p>Form Health’s Patient Care Coordinators (PCCs) provide comprehensive support for daily clinical operations, ensuring our patients have a smooth, caring, and well-organized experience from their very first interaction. Working collaboratively with medical clinicians and registered dietitians, PCCs support processes that enable excellent patient care. This includes a wide range of tasks from managing the patient's care team experience, coordinating patient scheduling, and assisting with benefits coordination, to answering questions regarding prescription fulfillment, and coordinating with external providers. PCCs will also provide essential support to clinicians by managing pre-visit lab work and inputting vital sign and medical record documentation. This role is 100% remote. </p> <p><strong>Where You’ll Deliver Impact:</strong></p> <p>We seek an experienced Medical Assistant, a collaborative team player with exceptional interpersonal and organizational skills, who is passionate about being part of a healthcare team and excited to work in a non-traditional telemedicine practice environment. Ultimately, your role is about being organized, thoughtful, and empathetic, making sure every patient feels personally supported at every step of their journey.</p> <p><strong>Key responsibilities include:</strong></p> <p><strong>Care Team Management:</strong></p> <ul> <li>Acting as the primary point of contact to maintain communication flow between patients and their care team.</li> <li>Receiving, triaging, and resolving incoming clinical messages from patients to clinicians, and relaying messages from clinicians to patients as requested.</li> <li>Conducting regular outgoing patient communications to ensure consistent engagement and optimize their success in the program.</li> <li>Providing routine administrative support for clinicians</li> <li>Contributing to special projects as directed, helping to improve our systems and make care more efficient and effective.</li> </ul> <p><strong>Lab / Medical Records Support:</strong></p> <ul> <li>Helping collect medical records from primary care or other providers when needed.</li> <li>Requesting medical records.</li> <li>Reviewing patient uploaded medical records.</li> <li>Facilitating Medical Records & Prescriptions, including sending lab requisitions.</li> <li>Assisting with prior authorization coordination and other prescription-related tasks.</li> <li>Assisting with medication refills and pharmacy questions. </li> <li>Supporting medical clinicians with pre-visit lab orders.</li> </ul> <p><strong>Membership / Program Status:</strong></p> <ul> <li>Guiding new patients through what to expect, both at the start and throughout their weight loss journey.</li> <li>Addressing general questions about the program.</li> <li>Supporting patients with account changes or cancellation requests.</li> <li>Providing feedback and communicating patient feedback to other company teams (i.e. Product, Engineering, Marketing) to improve operational tools and patient care.</li> <li>Assisting with clinical data collection as needed to track company’s key clinical performance indicators or for quality improvement purposes.</li> <li>Providing scheduling and appointment support for patients.</li> </ul> <p><strong>Billing / Insurance:</strong></p> <ul> <li>Assisting with general insurance and billing questions.</li> <li>Providing information regarding insurance coverage updates.</li> <li>Addressing billing concerns and coordinating with the billing department.</li> <li>Answering membership change questions related to billing.</li> <li>Providing information on the cost of memberships.</li> <li>Answering eligibility questions.</li> </ul> <p><strong>You’ll Be Successful in This Role Because You Have:</strong></p> <ul> <li>A Medical Assistant (MA) Certificate is required.</li> <li>At least two years of experience in a fast-paced primary care, obesity medicine, or endocrinology practice.</li> <li>Experience with prescription prior authorization processes is a plus.</li> <li>Excellent customer service skills.</li> <li>Excellent verbal and written communication skills.</li> <li>Highly organized with a proven ability to manage multiple tasks and time effectively.</li> <li>Proficient computer skills, with proficiency in platforms such as Slack and Google Workspace, and the ability to quickly adapt to evolving, custom-built IT systems.</li> </ul> <p><strong>More about FORM’s benefits:</strong></p> <ul> <li>Competitive salary and equity in a high growth start-up</li> <li>Comprehensive health benefits (medical, dental, vision) </li> <li>401k</li> <li>Flexible work schedules and paid time off</li> <li>Paid parental leave</li> </ul> <p><strong>FORM Health’s commitment to building a diverse, equitable, and inclusive work environment:</strong></p> <p>FORM Health is committed to creating a culture and environment that celebrates diversity and inclusion, while fostering safety and belonging.
