We are seeking an outgoing, empathetic, organized, and self-motivated Care Manager to join our growing team. In this role, you will play a vital part in engaging and supporting patients with a variety of health conditions through remote monitoring and ongoing communication.

At its core, this position is about building meaningful relationships with patients, understanding their symptoms, tracking changes in their health, and helping bridge communication between patients and their healthcare providers. You will interact with patients daily using structured questionnaires, assessments, and screenings designed to support proactive, patient-centered care. The work you do will be impactful, rewarding, and directly contribute to improving patient outcomes and the overall standard of care.

Key Responsibilities

  • Engage and connect with patients on a daily basis through remote communication
  • Administer pre-built health questionnaires, assessments, and screenings
  • Monitor and track patient-reported symptoms and health trends
  • Maintain knowledge and ongoing education related to common chronic and acute conditions
  • Collaborate with other Care Managers and clinical team members to coordinate care between patients and providers
  • Ensure accurate documentation and timely follow-up based on patient needs

Minimum Qualifications

  • Medical Assistant (MA) certification

Preferred Qualifications

  • Bilingual in Spanish
  • Case Management certification (CCM, ACM) obtained upon hire
  • At least 1 year of clinical experience in an HMO, medical group, affiliated model, hospital, or medical office/clinic setting
  • At least 1 year of experience in utilization management or case management

Competitive Pay ($15.00 - $18.00 USD / hour) with potential opportunity for $1K sign on bonus.
