In partnership with leading health systems, Cadence consistently monitors tens of thousands of patients to improve outcomes, reduce costs, and help patients live longer, healthier lives.</p></div><p><span style="font-weight: 400;">The Cadence Health team seeks a Registered Nurse Coordinator to appropriately monitor and triage patients based on vitals supporting the management of patient treatment plans and medications in collaboration with Cadence Nurse Practitioners and the patient's physician/ provider. <strong> </strong></span></p> <p><strong>This is a part-time position, scheduled for Saturdays and Sundays from 7:00 AM to 7:00 PM Eastern Time.</strong></p> <p><strong>WHAT YOU’LL DO: </strong></p> <ul> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Monitor patient vitals, symptoms, and labs to identify patients in need of clinical interventions. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Follow up with patients with abnormal readings to gather more information on their clinical status and triage appropriately. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Respond to inbound patient clinical questions and escalations by phone and text message. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Lead virtual follow-ups with patients to support program enrollment, treatment plan changes, medication adherence, and achievement of lifestyle goals. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Support clinical documentation and development of care summaries for patients' physicians.</span></li> <li>Be in charge of making sure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.</li> <li>Support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF, hypertension, and other chronic conditions like type 2 diabetes.</li> <li>Be instrumental in shaping the culture of one of the fastest-growing teams at Cadence. </li> </ul> <p><strong>WHAT YOU’LL NEED</strong><span style="font-weight: 400;">: </span></p> <ul> <li><span style="font-weight: 400;">4+ years experience treating CHF, hypertension, and Diabetes patients either in an outpatient or inpatient setting. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Compact multi-state licensure (RN compact).</span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Experience working in a CHF bridge clinic environment. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Experience working with remote patient monitoring technology. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Passion for the patient/ customer experience and systematically improving healthcare with digital innovation. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Independent thinker/ operator. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Ability to monitor patient vitals, symptoms and labs to identify patients in need of clinical interventions. </span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Ability to follow up with patients with abnormal readings to gather more information on their clinical status and triage appropriately.</span></li> <li><span style="font-weight: 400;"> </span><span style="font-weight: 400;">Ability to lead virtual follow-ups with patients to support program enrollment, treatment plan changes, medication adherence and achievement of lifestyle goals. </span></li> </ul> <p><strong>WHO WE ARE:</strong></p> <p>Cadence Health was built around a simple promise: patients always come first.
