Saint Therese at Oxbow Lake is looking for full-time Infection Preventionist/MDS Coordinator (RN) to join our dedicated team! This position will be scheduled to work day hours Monday - Friday (80 hours per pay period). This blended role will be responsible for: As Infection Preventionist evaluating the quality of resident care and resident outcome as they relate to healthcare associated infections; collects prepares and analyzes HAI infection and surveillance data; presents infection data and makes recommendations for actions. The Infection Preventionist also monitor employee compliance in use of PPE and use of transmission precautions barriers and infection prevention measures; prepare and presents educational offerings for the staff and services as recourse to all departments and personnel. As an MDS Coordinator you will oversee and complete MDS and care planning. Complete all documentation for RAI and MDS assessments and observations. Develop initial and temporary care plans, and attend resident care conferences, manage and assign tasks to nursing staff on the assigned unit, and work in collaboration with Admissions staff and nursing team. Qualifications • Leadership and communication skills Education: Current Minnesota Registered Nurse Licensure in good standing.. Experience: Minimum of two (2) years of long-term care experience is required. Previous education, training, experience, or certification in infection control and prevention is required. Previous experience in completion of MDS process is required.
