Job Description Summary

The Primary Care Nurse Practitioner (NP) in a value-based clinic delivering comprehensive, patient-centered care with a focus on quality, efficiency, and population health outcomes.

Primary Care Provider, based in a clinic with a small portion of telehealth. Role expectations include:

1) Comprehensive Patient Assessment and Evidence-Based Management

  • Conduct thorough patient evaluations, including history, physical exams, and risk stratification, to diagnose and manage acute and chronic conditions.

  • Implement evidence-based care plans, utilizing clinical guidelines and population health data to optimize outcomes and reduce unnecessary utilization.

2) Care Coordination and Team-Based Collaboration

  • Actively participate in interprofessional care teams, coordinating with physicians, care managers, pharmacists, behavioral health, and social services to ensure seamless, patient-centered care.

  • Facilitate transitions of care, follow-up on referrals, and communicate care plans across the team to minimize fragmentation and improve quality.

3) Proactive Population Health Management

  • Identify and engage high-risk or rising-risk patients using registries and data analytics, performing outreach, preventive care, and chronic disease management to close care gaps and reduce avoidable hospitalizations.

  • Monitor and track quality metrics (e.g., HEDIS, CMS Star Ratings), implementing interventions to meet value-based performance targets.

4) Patient Engagement, Education, and Self-Management Support

  • Educate patients and families about diagnoses, treatment options, preventive strategies, and self-management techniques, empowering them to participate in their care.

  • Utilize motivational interviewing and shared decision-making to improve adherence, address social determinants of health, and support lifestyle changes aligned with value-based goals.

Requirements:

  • Requires an MS in Nursing

  • Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.

  • Current unrestricted RN license and NP license in applicable state required.

  • Satisfactory completion of a Tuberculosis test is a requirement for this position.

**This position is bonus eligible based on individual and company performance.**
