About Town Square Health

Town Square Health is a value-based, multi-speciality, provider-owned practice, with a bold vision: to create the gold standard for how we experience healthcare in the United States. Our model was built by David Buchanan, a prior Oak Street Health executive and practicing internal medicine physician with 25 years of experience in value-based care serving Medicare and Medicaid patients.

Our breakthrough care model for Medicare patients combines relationship-driven primary care and specialty care into simultaneous visits. Each patient is supported by a dedicated Medical Assistant – a concierge partner who builds long-term trust, eases caregiver burden, and leverages AI-enabled workflows to improve chronic disease outcomes.

The Opportunity

Town Square Health is hiring a Primary Care Provider (NP) in the Rochester area to deliver high-quality primary care in a Medicare-focused, value-based care environment. This is a clinic-based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40-minute visit lengths based on patient needs, and a team-based care model with two Medical Assistants paired with each provider.

You will play a key role in delivering excellent clinical care, coordinating across the care team, and helping shape a new care model designed to improve outcomes and patient experience.
